Please bring an official document issued by a public authority that clearly indicates the relationship between the applicant and the head of household (e.g., family register copy/extract, birth certificate, marriage certificate, etc.). In the case of a moving-in notification from abroad, or if the "個人番号" (Individual Number / My Number) is not listed on the moving-out certificate, a request to generate an Individual Number must be made to the 地方公共団体情報システム機構 (Japan Agency for Local Authority Information Systems) upon moving in.
